Sometimes, I just do not really think about these questions when I want to get SEO agent.. I agree with some ofother comments for number 5 as I also imagine that the agent must be very reluctant to reveal the details of their clients. But for the rests of the questions, they make perfect sense that we really need to forward the agent these question if we aim to achieve the best result. The questions which you have mentioned in your article are nice and would be really helpful when hiring an SEO agency,all are logical and good, But i would not agree with the fifth one as no company could be able to explain that.The best way is to go through the agency site and if it is listed in first three pages of the search engine then its of sure that they know their work.
